“DBS means everything – for some time”. Patients’ Perspectives on Daily Life with Deep Brain Stimulation for Parkinson’s Disease

چکیده
Background: Deep brain stimulation (DBS) is an established treatment for Parkinson’s disease. However, patients ́ own perceptions of the impact of DBS on their daily living is not fully explored. Objective: We aimed to collect and analyse patients’ narratives about their everyday experiences of being on chronic DBS. Methods: Semi-structured interviews with open-ended questions were conducted with 42 patients (11 women) who had been on DBS for a mean of three years. The questions were related to patients’ ordinary daily life and eventual changes, both negative and positive, brought about by DBS. The interviews were transcribed verbatim and analysed according to the difference and similarity technique in grounded theory. Results: From the patients’ narratives the core category ‘DBS means everything – for some time’ was established, and supported by the following categories: 1) Relief from invasive tremor. 2) A rescue from cramps and pain. 3) Easier movement swings and more predictable living space. 4) Hard, but compared to previous suffering, bearable adverse events. 5) Parkinson’s disease is progressing despite DBS. Conclusions: The analysis of the participants’ narratives shed light on patients’ unique perceptions and perspectives of the impact of DBS on their everyday lives. Patients with advanced PD highly appreciated the positive impact of DBS on their daily life even if this impact is limited in time. For the majority, the relief from the severe parkinsonian symptoms, especially tremor and painful cramps, outweighed the side effects of DBS. The study provided information not readily captured by pre-formulated questionnaires and scales.

The effect of bilateral subthalamic nucleus deep brain stimulation (STN-DBS) on the acoustic and prosodic features in patients with Parkinson’s disease: A study protocol for the first trial on Iranian patients
Background: The effect of subthalamic nucleus deep brain stimulation (STN-DBS) on the voice features in Parkinson’s disease (PD) is controversial. No study has evaluated the voice features of PD underwent STN-DBS by the acoustic, perceptual, and patient-based assessments comprehensively. Furthermore, there is no study to investigate prosodic features before and after DBS in PD. The curren...

Introduction: Parkinson’s disease is one of the most disabling diseases which by electrode implantation and stimulation of subthalamic nucleus (STN), much progress has been made in the treatment of drug resistant patient. This new method of neurosurgery may have some neuropsychological side effects on the patients. The main aim of this study is to evaluate the effects of this kind of treatment ...
متن کاملTreatment of Neurological and Psychiatric Disorders with Deep Brain Stimulation Raising Hopes and Future Challenges
The technology of Neural Stimulation in recent years has become the focus of the research and treatment, although it has been around for many years. The potential use of stimulating the brain and nerves ranges from the spinal cord stimulation to the implantations of cochlear and bionic eyes with a large discrepancy between the clinical readiness for these various uses. Electrical high-frequency...
متن کاملRelationship of the P Angle with Stereotactic Arc in Intraoperative Outcome of Patients with Parkinson’s Disease Undergoing Deep Brain Stimulation
Background & Aim: DBS (deep brain stimulation) is a new and successful technique in treatment of symptoms of Parkinsonism especially after drug resistance. Research in this field is mostly designed for evolution of this technique. The present study aimed at evaluating the relationship between the angle formed in midsagittal and STN (sub-thalamic nucleus) axis line and recording l...
متن کاملAnatomical situation of the subthalamic nucleus (STN) from midcommissural point (MCP) in Parkinson\'s disease patients underwent deep brain stimulation (DBS): an MRI targeting study
Abstract Introduction: It is demonstrated that the degree of clinical improvement in Parkinson's disease (PD) achieved by deep brain stimulation (DBS) is largely dependent on the accuracy of lead placement. In addition, individual variability in the situation of subthalamic nucleus (STN) is responsible for spatial inter-individual fluctuations of the real patient's target. Objecti...
